Procedures for Completing "Leaving Program"

1) If possible, conference with student to determine reasons for leaving.

2) If you know of any resources the student could access to help with the problem, discuss with student and complete appropriate space on form.

3) Determine when the student plans on returning. Have the student complete form.

4) Go over form and encourage the student to monitor the recommended strategies.
